(a) The delay of death gratuity in respect of Ms. Asumpta Ndambuki - P/No.1990135188 was caused by the unavailability of two supporting documents that is, Death Certificate and Birth Certificate of her children. The two documents have been submitted and the death gratuity has been processed and forwarded to the Director of Pensions. In respect to Ms. Fillister Ndunge Ngumbi - P/No.1990081177, a letter was written to the next of kin on 24th February 2004 requesting for the submission of the following documents: (i) Original Death Certificate. (ii) A letter from the chief detailing the legal dependants and their permanent postal addresses. (iii)Birth certificates of the children. (iv) Copy of national ID cards of the legal representatives."
