Some even die or get injured and yet there are no sanctions against such institutions. That is part of the risk occurrences that we need to point out, so that any agency that causes harm to people by creating a risk and not providing a proper mechanism through which you ensure that you do not put the lives of others at risk while undergoing either repairs or creating something; we need to sanction such institutions.
