If you look at the definitions at the second paragraph on Page 1329, there is the definition of a specialist there. It means “a medical practitioner or a dentist who has completed an approved postgraduate training in a particular field of medicine or dentistry.” And Section 3A(1)(i) is talking about a specialist medical or dental practitioner.
